Dr. Kozarek has been Tara’s Medical Director since June 1996. He is a graduate from the University of Wisconsin Medical School and is a Board-Certified Family Physician.

Dr. Kozarek is a member of the Indiana Medical Association, the American Academy of Family Physicians and the American Board of Family Practice.

He is affiliated with Johnson Memorial Hospital, Franklin, Indiana and Columbus Regional Hospital, Columbus, Indiana.
